| Fee schedule for uninsured Initial evaluation/ consultation/ treatment: 90 min : $150.00 Follow up visits: 60 min : $120.00 Insurance: We do accept most PPO's and some POS insurance plans, call, fax or e-mail your insurance information with your date of birth. We will check to see if your insurance has alternative medicine benefits. Cancellation Policy If you need to reschedule please be sure to call the clinic 24 hours before your scheduled appointment. If you do not cancel or reschedule at least 24 hours in advance- or if you miss your scheduled appointment you will be charged a cancellation fee of $80.00. We accept cash and personal checks. |
